COVID-19 update for family health team patients

Email bookings
We are happy to share that our email booking option is now accepting virtual appointment requests.
- Email FHTBookings@smh.ca to schedule a virtual visit (telephone or video).
- In order to book an appointment by email you must have an email consent on file.
- If you haven’t already done this, please ask your doctor to complete an email consent at your next visit.
- When sending an email booking request please include your full name, date of birth, phone number, family doctor’s name, clinic location and reason for visit.

We do have access to interpretation services for telephone visits.
Urgent health care issues
- Please call us if you have urgent health care issues. We continue to have 24-hour telephone access.
- If you are having a medical emergency call 911 or attend the emergency room.
COVID-19 symptoms and testing
- COVID-19 testing criteria has changed. Testing has been expanded to include ALL people with COVID-19 symptoms, and those without symptoms who are concerned they have been exposed to someone with COVID-19.
- For additional direction you can complete the provincial COVID-19 self-assessment.
- If you need testing we recommend that you visit one of the COVID-19 Assessment Centres
- Please visit Toronto Public Health for updates on COVID-19 symptoms and what to do if you would like to get tested, including the locations of COVID-19 Assessment Centres.
- If you are sick with routine cold symptoms, you likely do not need to come into the clinic to be seen. If you are concerned and need some direction or advice or if you have tested positive for COVID-19, please call us to speak with a health care provider over the phone.

Weekend Urgent Care Clinics
We are still here to provide you with care over the weekend.
Please call us first to be prescreened over the phone. You will be able to speak to a member of our care team who will complete an assessment and provide further direction. If it is decided that you do need to come into clinic on Saturday or Sunday you will be given an appointment time at the time of your call.

If you need medical care during regular business hours, on the weekend, or after-hours you can call your clinic to speak with a member of our team.
If you are calling after 5 p.m., and have an urgent medical issue that is not a medical emergency, you can contact the St. Michael’s Locating department and ask to speak with the family medicine resident doctor on call. You can reach locating by calling your clinic, and pressing “1” to be connected.

Prescription renewals
Please ask your pharmacy to send prescription renewals by fax. Your pharmacist may also be able to extend prescriptions for some chronic medications.
