About us


As a patient in our academic Family Health Team, you will receive excellent care that reflects the values and mission of St. Michael’s Hospital. Your care will be delivered by our skilled interprofessional team and will be centered on your needs. The Department of Family and Community Medicine is dedicated to improving care both in and out of the clinic through quality improvement and research programs. As part of our affiliation with the University of Toronto, we are a training site for students from medicine, chiropractic care, psychology, nursing, dietetics and pharmacy.

What is a family health team?

Family health teams are primary health care organizations, funded by the Ontario Ministry of Health and Long-Term Care, that include a team of family physicians, nurse practitioners, registered nurses, social workers, dietitians, and other professionals who work together to provide primary health care for their community. They ensure that people receive the care they need, as each team is set-up based on local health and community needs. By coordinating the different types of health professionals needed to address your health concerns, our team ensures that you quickly receive the care that best promotes your well being.
