Specimen Handling Collection Instructions: Please see the Newborn Custody and Control Instructions Form. and Meconium Drug Panels. The name of CAS worker and CAS location must be provided with the sample. Laboratory Staff Instructions: Ensure all requisitions and forms are completed properly. The sample Collector (nurse or physician) must print, sign and date the form. The Technician must print, sign the processor certification They are also required to seal the sample and container with the specimen seal control number. Ensure the correct drug panel is ordered. Do not sign the Sendouts Section Certification. It is for the ICL staff to fill. (I). NPDSE = New Perinatal Drug Screen 5 Group, Meconium + ETOH (II). NPDS5 = New Perinatal Drug Screen 5 Group, Meconium (III). NPDH5 = New Perinatal Drug Screen 5 Group, Hair (IV). MISCS = All other Meconium Drug Panels (Enter these tests in the comment section). Tests cannot be ordered individually. The clinician must specify if they are requesting panel 5, 7, 9, 12, 13, or 14. Enter the name of CAS worker and CAS location in the order entry comment section. Collect and receive sample according to the last collection date and time. Complete the Referred out sendout requisition. Copies of all requisition must be filed with sendout bench and team lead's record file. Rejection Criteria: unlabeled or samples of non sufficient quantity. Transportation: Send sample to core lab as soon as possible.
