Services

Direct Patient Care

Our pharmacists are fully integrated members of the patient care team, working to optimize patient care by identifying, preventing and resolving drug related problems. We provide care for both inpatients and outpatients.

Our pharmacists also promote the safe and effective use of medication through medication education and counselling, initial and on-going patient assessments, medication interviews. We also assist in the development of tools to support best practice in medication usage, including drug administration, drug use guidelines and protocols.

Education

Our pharmacists provide education to our health care teams of medical residents, nurses, dieticians, physiotherapists and students through presentations, physician rounds, one-on-one teaching sessions and experiential teaching.

Some pharmacists are involved in teaching programs external to the Hospital, such as the Leslie Dan Faculty of Pharmacy, nurse practitioner program, chiropody and Michener Institute.

Services: Drug Distribution

The pharmacy department distributes medications for admitted patients using individually prepared and labeled “unit dose” 24-hour supplies of medication. We also prepare medications that require compounding under aseptic conditions to maintain their sterility, including chemotherapy, intravenous nutrition, and other specialty medications.

All medications are stored, prepared and dispensed to meet legislated and professional practice standards and to ensure safety and accuracy.

The research pharmacy will dispense all medications provided to patients as part of a clinical trial approved for the Research Ethics Board.

Services: Drug Information Services

To promote the optimal use of medication within the hospital, our drug information pharmacists respond to questions from St. Michael’s staff and physicians. In collaboration with physicians, nurses and pharmacists, drug information pharmacists develop tools to promote best practices in protocols and guidelines for drug use, drug formulary, drug utilization reviews and our quarterly drug information bulletin.

If you are a patient with a drug information question and your regular pharmacist is not available, you may call Telehealth Ontario toll free at 1-866-797-0000. Your question will initially be screened by a registered nurse, but pharmacists are available 24 hours a day to respond to medication related questions.
