Urgent Medical Care Services



Effective Monday March 23, 2020

The weekday hours of operation for the St. Michael's Family Health Team (FHT) will be 9 a.m.- 4:30 p.m.

The FHT evening walk-in urgent care clinics will be closed until further notice. After 5 p.m., patients can call 416-864-5431 to speak with the family medicine resident doctor on call.  

Patients who have scheduled evening clinics with their regular doctor should be aware that these appointments will now be converted to phone visits and should expect to receive a phone call from their physician sometime during the evening in which the appointment is scheduled. Routine or non-essential appointments may be rescheduled to a later date. We appreciate your understanding.

In an effort to reduce risk to yourself and the public, the St. Michael’s Hospital Academic Family Health Team is reducing office visits. As recommended by the Ontario Medical Association, starting today we are changing most appointments to over the phone or virtual visits. Please do not come to the clinic unless you have a pre-booked appointment.

Shift to virtual care appointments

In order to prevent the spread of COVID-19, starting immediately, we will be rescheduling all non-urgent in-person patient appointments until further notice. To continue to provide the best possible care to our patients, we will be offering virtual care visits such as telephone appointments and video visits (eVisits) to patients to address their health concerns.

Family Health Team (FHT) visitor guideline

Due to increasing COVID-19 community transmission and recommendations from the Ministry of Health we are limiting visitors at the FHT.  If you are a patient and are coming in to the clinic, we are only allowing people described below to come with the patient:

  • A caregiver
  • A support worker
  • One parent/guardian for patients under the age of 16
  •  A support person helping with language barrier

All other people will not be allowed to come with the patient to their clinic visit.

Thank you for your understanding in this matter.  This decision was made to slow the spread of the virus and follow social distancing practices.


Our Family Health Team is committed to supporting your health care needs.

During regular clinic hours, our Family Health Team is available to help you with urgent illnesses or injuries that require same-day assessment. Our regular clinic telephone hours are from 8:30 a.m. to 5 p.m. If you are not sure if your concern is urgent, please call anyways! We are happy to help determine the most efficient way for you to get care.

If you experience a serious life-threatening medical emergency, always go to your nearest emergency department or call 911.

What is NOT an urgent or same-day concern?

  • Prescription renewals: your pharmacy can fax a request to your provider or provide you with a bridging supply of non-narcotic medications
  • Forms: these should be dropped off in clinic for your regular provider to complete (please provide two weeks for completion)

After hours clinic locations for urgent care

If you have an urgent health concern outside of regular clinic hours (on evenings, nights or weekends) - we can help you! It is safer to be seen by your family health team at our after-hours clinics instead of another walk-in clinic because our team members can access your full medical history and quickly alert your regular provider that you were seen. As a patient of the St. Michael’s Hospital Academic Family Health team you can attend any of the after-hours clinics listed below regardless of where you receive your regular care!

Regular hours (all locations)

Monday - Friday, 9 a.m - 4:30 p.m.

Urgent care (weekends only)

Saturday - St. James Town Health Centre
9 a.m. - noon and 1 - 4 p.m., last check-in at 3:30 p.m.

Sunday - Sumac Creek Health Centre
9 a.m. - noon and 1 - 4 p.m.,
last check-in at 3:30 p.m.

Need to speak with a doctor after-hours or on weekends?

We provide 24 hours/day access!

Call any of the clinic numbers above and follow the prompt for “after hours” or “urgent care” outside of regular clinic hours. During the clinic hours listed above your call will be routed to the reception desk of the after-hours clinic. If the after-hours clinic is finished, your call will be transferred to the Locating Department at St. Michael’s Hospital at which point you should:

  • Request to speak to the family doctor on call
  • Provide your name, clinic, doctor and phone number
  • Stay off the phone and you will get a call back in 15 minutes from the doctor on call.
