What is MyChart?

MyChart is a secure, online portal that shows patients their own hospital records. It was developed at Sunnybrook Hospital and is now used in hospitals across Canada.

MyChart can show you your:

  • Lab results (e.g. blood tests)
  • Medical imaging reports (e.g. summaries of x-rays and ultrasounds)
  • Notes written in your hospital file
  • Instructions that were given to you when you were discharged from hospital (for inpatients only)
  • Upcoming appointments

Who can use MyChart?

To use MyChart, you must have an email address, an Ontario Health Card and received care at St. Michael’s at any point since Jan. 1, 2017.

Who can access my MyChart profile?

When you first register for MyChart, only you will be able to see your health records. After you register, you can choose to give other people access to your MyChart records. This is so that people who care for you, such as a family member or family doctor, can use your information to get a better picture of your health.

How do I register for MyChart?

To sign up for MyChart, you must come to the hospital in person with your OHIP card. This step can’t be done by email or phone.

There are two ways to sign up:

  1. At Patient Registration on the first floor of Donnelly Wing South, one floor above the Queen Street entrance.
  2. At your next outpatient clinic appointment – just ask at reception. Please note that our Family Health Team can’t sign patients up for MyChart at this time.

Our staff will ask for your email address, OHIP card and photo ID. Next, we’ll send you an email with a MyChart link and PIN number. The email will come from mychart@sunnybrook.ca. Click the MyChart link in the email, and follow the instructions. Once you complete your registration online, you can log in at www.mychart.ca anytime to look at your health record.

How is my information entered into MyChart?

Most of your information is sent to MyChart automatically, as soon as it’s entered into the hospital’s electronic medical record system. You can also enter some information yourself. For example, MyChart features an address book to help you keep your different care providers’ addresses and phone numbers in the same place.

How does MyChart make sure that my information stays safe and confidential?

MyChart is secure like online banking; it uses the same encryption technologies used by the major banks.

I logged into MyChart but my test results aren’t there. Why?

Although most test results will show up as soon as the results are available, some might not be entered until reviewed directly by a physician. Ask your care team when you can expect to see your results.

If you are looking for an older test result, please note that only test results generated after Jan. 1, 2017 and medical notes generated after July 1, 2017 are included in MyChart.

I’m already registered for MyChart through another hospital. Why can’t I see my information from St. Michael’s?

Before St. Michael's can put your health information into MyChart, we need your permission. To do this, you must sign up at Patient Registration on the first floor of Donnelly Wing South (one floor above the Queen Street entrance), or at your next outpatient clinic appointment – just ask at reception. Please note that our Family Health Team can’t sign patients up for MyChart at this time.
