How can I make an access to information request?

Determine what records you need by reviewing our directory of records; our directory of what business records are available on our website; or contacting the information access and privacy office at St. Michael’s at 416-864-6088 or by e-mail at fippa[at]smh.ca. We will assist you in determining whether or not the information is already accessible to the public. If the information you are requesting is not already routinely available, complete an access request form. To learn more about making an access request, visit our access request page.

How much does it cost to make an access request?

A $5 application fee is required for all access requests. Additional fees may be applicable. For a detailed fee breakdown, visit our access request page.

What records can I request under FIPPA?

Operational or business records, including administrative or financial records held by the hospital on or after January 1, 2007, may be requested as part of an access request under FIPPA. Can I submit an access to information request for personal health information?

Requests for personal health information should be made to the health records office at St. Michael’s via our Contact Us page. Personal health information is governed by an act known as the Personal Health Information Protection Act (PHIPA).

How long does it take to get the information after submitting an access request?

St. Michael’s will respond within 30 days of receiving an access request. Depending on the type of information you are looking for or the length of time it may take to collect the information, a time extension may be required, as allowed under the Freedom of Information and Protection of Privacy Act (FIPPA).
