Virtual Hub as a resource for improved patient care coordination

You may access services for improved care coordination for patients with complex health needs from the Mid-East Toronto Sub-Region Primary Care Team webpage.

Referring your patient to the Virtual Hub provides urgent access to a Transitional Care Coordinator (TCC) who will work with you and your patient’s existing care team to facilitate greater coordination and collaboration. The TCCC will work with your patient and care team to identify gaps in service and address these gaps by developing collaborative plans and accessing new resources. This model of intensive care coordination results in a shared understanding of your patient’s conditions across providers, smoother transitions in care, greater patient involvement and ongoing communication between providers from diverse sectors, leading to reduced avoidable visits to Emergency departments and improved health outcomes for patients.
